Screening for chronic diseases (cardiovascular risks, type 2 diabetes, hypertension)

Eligibility

• For individuals age 18 to 75 who are in good health, without a diagnosis of diabetes, heart disease or hypertension, whether or not they have a family doctor.

You don’t need to travel for your appointment. During your appointment, a nurse will conduct a clinical assessment by phone. They will confirm whether you’re eligible for the service and, if so, determine the appropriate screening test for your situation.
