Screening for chronic diseases (cardiovascular risks, type 2 diabetes, hypertension)
Diabetes and cardiorespiratory diseases can occur without any symptoms and are among the leading potentially preventable causes of death. Prevention relies on early detection and prompt management to avoid serious complications.
Eligibility
For individuals age 18 to 75 who are in good health, without a diagnosis of diabetes, heart disease or hypertension, whether or not they have a family doctor.

You don’t need to travel for your appointment. During your appointment, a nurse will conduct a clinical assessment by phone. They will confirm whether you’re eligible for the service and, if so, determine the appropriate screening test for your situation.
