Who is eligible?
Home support services are intended for people with reduced autonomy due to illness, ageing or a disability and for loved ones who live with or take care of a person with limited capacities. These services are also for people who have just had surgery or have been discharged after a stay in hospital and who need assistance during their convalescence.
The services are offered in the user’s home, in a private seniors’ residence or in an intermediate resource. Following an assessment, a support worker will be assigned to coordinate the services. A needs assessment and service plan are developed with the participation of the user and their loved ones.
