Accessing Care Without a Family Doctor
The Primary Care Access Point (GAP) allows people without a family doctor to obtain a medical appointment or health service. Starting now, to receive care more quickly, you can fill out an online questionnaire to indicate your health needs.
If you need help completing the digital questionnaire, call 811, option 3. A GAP staff member will call you back to assess your needs and direct you to the most appropriate resource.
When your call is received, you can choose one of the following options:
- Accept the call.
- Request a call back later.
- Cancel your request if it’s no longer needed.
GAP will make up to three attempts to reach you.
This process allows you to speak with the person best equipped to respond to your needs while avoiding time on hold.
If your life is in danger or your health condition requires immediate attention, you can at any time:
- dial 911 for an emergency response or ambulance transport;
- go to the nearest hospital emergency department.
Transitional services
You have no immediate health problem but may have risk factors for a chronic disease like diabetes or cardiovascular disease? A nursing assessment is available to screen for certain chronic conditions preventively.
You can contact the GAP to see if this assessment is relevant for you. Do you have a chronic disease diagnosis (diabetes, high cholesterol, cardiovascular disease) and want active follow-up? Nursing follow-up is available under certain conditions. You can contact the GAP to check your eligibility.
