3 easy steps to participate in the program!

1. You must be referred by your family doctor. If you don’t have a family doctor, sign up with the Family Doctor Finder or go to a walk-in clinic. 

Our professionals follow patients for the following chronic illnesses:

Metabolic diseases

  • Dyslipidemia
  • Prediabetes
  • Diabetes
  • Obesity
  • Metabolic syndrome
     

Cardiovascular diseases

  • Angina
  • Ischemic heart diseases (heart attack, bypass)
  • High blood pressure
  • Heart failure
  • Peripheral arterial disease

Lung diseases

  • Asthma
  • COPD

Rheumatic diseases

  • Arthritis
  • Hip and knee arthrosis
  • Fibromyalgia
  • Persistent pain 
     

2. Once our teams have received the referral from your doctor, someone will contact you to explain how you can  watch the video clips (in French) on healthy lifestyle habits, nutrition and arthrosis or to give you an appointment with a professional depending on your needs.  
 
3. You must then fill out the video viewing acknowledgment form.

 

Teaching and follow-up

The chronic disease program is offered by a group of professionals (nursing staff, nutritionists, kinesiologists, respiratory therapists, physiotherapists, social workers, etc.) who provide teaching and follow-up for the different chronic diseases and their associated risk factors.

The services are designed to increase your autonomy while improving your knowledge of the disease so that you can better control the symptoms, risk factors, medication and your return to daily activities. The objectives are to:

  • encourage healthy lifestyle habits;
  • prevent long-term complications;
  • promote self-management by giving you the tools to ensure healthy management of the disease.
