Registration of birth

After the birth of your child, the staff in the obstetrics department or the midwife will give you a copy of the declaration of birth form.

The CISSS will send three copies of this document to the Direction de l’état civil.

The Direction de l’état civil will send you a confirmation of the registration of the birth, within three months of the birth, along with an Application for a Certificate or Copy of an Act of Birth. During this period, you’ll receive your child’s health insurance card.
